Request Management System

The Request Management System is a web-based system for faculty and staff to use when requesting university services.  With this system, users can submit requests to the department via the online form, track requests, retrieve reports or files from the online system, and receive automatic acceptance and completion notifications. 

Currently, these office are set up to accepts requests via the online system.  Other business units may be added in the future.

Please refer to the 'Data/Survey Request Procedures' to help with the submission process.

To access the Information request and tracking system, you will be required to enter your DSU network ID (dsu\xxxxx) and password.

Data & Survey Request

To better serve you, please complete one of the forms below to request data or to request authorization to administer a survey to DSU faculty, staff or students.

For Current DSU Faculty, Staff & Students

  • Request for Data
    • Complete the data request form under the Data/Survey Business Unit to request institutional data.
  • Request for Survey Development & Administration
    • Complete the Survey Request Form under the Data/Survey Business unit
    • A staff member in the Office of Institutional Effectiveness & Assessment will help with the survey development, creation, administration, and analysis, if needed.
    • All survey requests must be approved by DSU's Vice President's Council.

For non-DSU Personnel

  • Request for Survey Administration
    • Complete this form to request permission for survey administration to DSU faculty, staff and/or students.
    • All survey requests must be approved by DSU's Vice President's Council.


Administrative Programming Unit of Information Technology Services (ITS)

ITS Administrative Programming

Use this for any request that you previously sent to DSU-Computing Svs-Prog Req group email box.  This includes such requests for new or routine reports, daily processing needs, menu creation, prospect loads, duplicate records, Colleague ID/password or printer resets, etc.  

  • Service Requests
    • Complete the request for services form under the ITS Administrative Programming business unit
  • ID/Printer Reset Requests
    • Complete the ID/Printer Reset form under the ITS Administrative Programming business unit
    • Use this for problems encountered with existing system credentials including Colleague, EMAS, Banner, R25, etc. or for Colleague printer problems.
