If you see behavior that does not align with our values, report it. Use the appropriate form to submit your concern. Your report helps keep DSU a place where people are treated fairly and with respect.
If you are experiencing—or are aware of—an emergency where someone’s immediate health, safety, or property is at risk, please call 911. These reporting forms are not monitored in real-time and are not intended for emergency use. Reports submitted through these channels are typically reviewed by DSU staff within one business day.
Academic integrity
Academic dishonesty is any type of cheating, plagiarism, fabrication or falsification of data, deception of an instructor by providing false information for missing an exam or deadline or claiming to have submitted coursework when you know you have not.(SDBoR poliy: 3.4.2; 2.1) Faculty may use this form to report instance of academic misconduct such as cheating, plagiarism, data misrepresentation, or any other violation as defined in the Student Code of Conduct.

Title IX
Dakota State University is committed to maintaining a safe and respectful campus environment, free from harassment, discrimination, and all forms of sexual misconduct. This includes, but is not limited to, sexual harassment, sexual assault, dating violence, domestic violence, stalking, and retaliation. This reporting form is intended for incidents involving allegations against students related to the above behaviors.
Definitions
Harassment/Retaliation: Harassment is unwelcome conduct based on protected characteristics (like race, sex, or disability) that creates a hostile, intimidating, or offensive environment.
Retaliation is punishing someone for reporting misconduct, filing a complaint, or participating in an investigation.
Sexual Misconduct: Sexual misconduct is any unwelcome sexual behavior that includes actions like harassment, assault, or coercion, and violates a person’s boundaries, rights, or consent.
Hazing: Hazing is any act, whether physical, mental, emotional, or psychological, that is required to join or participate in a group and causes harm, humiliation, or risk—regardless of the person’s willingness to participate.
Student conduct
Please use this form to report potential violations of the South Dakota Board of Regents Student Code of Conduct, which governs the behavior of students and recognized student organizations both on and off campus. Incidents may include, but are not limited to: alcohol misuse, drug use, violations of Residence Life or other campus policies, physical assault, misconduct by organizations, disruptive behavior, or theft.

Risk management
Accident and injury reporting is imperative to maintaining a safe campus. Reporting any near misses or concerns can aid in prevention of potential injuries. Students, visitors, and employees are encouraged to use the page for reporting a safety concern online to the Risk Management Office.
Concerns/Complaints
DSU is committed to fostering a safe and welcoming campus and online environment for all students. Use this form if you are a student and would like to file a complaint or share your concerns. The Provost Office will receive the complaint and route it to the appropriate department or office. This form can be completed for academic and non-academic concerns or complaints.
