Many forms require DSU login to access. If you are needing a copy of a specific form and you do not have login information, please contact the DSU Registrar's Office for assistance.
Changing Your Name
Current and former students can request to change their legal and/or preferred name. Faculty or staff members within DSU or another SD Board of Regents campus must submit a name change request with their campus Human Resources department.
Changing your Address
1) Go to MyDSU 2) Login to SNAP For Students 3) Select 'My Academic Information' 4) Select 'Student Personal Information' 5) Select the pencil icon next to each address type you want to update. If you need to remove an address type, select the trash icon.
